Medicare, most Managed Care Organizations and private insurers offer some type of coverage for breast forms, pocketed bras and camisoles. Medicare does not cover wigs or compression sleeves but some private insurers do.

We encourage you to call your insurer before your appointment so you know what your benefits are. If you are unsure of your coverage, we will be happy to help you.

Private Insurance

LifeChanges Boutique, Inc. participates with the following private insurance plans:

• Highmark Blue Shield
• Capital Blue Cross
• South Central Preferred
• Aetna
• DMEnsions

We will also submit claims for patients to out of network providers.

Medicare

LifeChanges Boutique, Inc. accepts Medicare and is required by law to file claims for individuals who purchase any items that are covered by Medicare. As of January 1, 2004, Medicare will cover 1 breast prosthesis or 1 leisure breast prosthesis every 2 years and mastectomy bras based on medical necessity.

Medicaid

LifeChanges Boutique, Inc. accepts Medicaid.
